Aisha Grover is 24 and has noticed dark circles since her teenage years. They show up in photographs regardless of how well she sleeps. Her mother, Sunita Grover, 54, developed a different under-eye concern much later: increasing hollowing and shadowing after menopause. They both describe the problem as “dark circles”, but the biology behind their concerns is not the same — and neither should be their treatment.

Why Dark Circles Are Not One Single Condition

“Dark circles” is a visual description, not a single diagnosis. The under-eye area can look dark because of increased pigment, very thin skin through which blood vessels show, a tear-trough hollow that creates a shadow, puffiness that alters how light falls on the area, or a combination of these factors. Allergies, eczema, repeated eye rubbing, sun exposure, genetics and age-related structural change can all contribute.

This is why two people of the same age — or even two members of the same family — may need completely different treatment plans. The right treatment starts by identifying the dominant cause.

What Type of Dark Circles Do You Actually Have?

1. Pigmentary dark circles — These usually appear brown to grey-brown and may be influenced by genetics, sun exposure, repeated rubbing, eczema or post-inflammatory pigmentation. Treatment focuses on pigment control, barrier care and sun protection.

2. Vascular or thin-skin dark circles — These can look blue, purple or dusky because vessels and deeper structures are more visible through thin under-eye skin. Skin quality and vascular visibility matter more here than pigment alone.

3. Structural dark circles or tear-trough hollowing — A depression between the lower eyelid and cheek creates a shadow that can look like pigmentation even when the skin itself is not especially dark. This is a volume-and-anatomy problem rather than simply a colour problem.

4. Puffiness-related dark circles — Under-eye bags or temporary fluid retention can create shadowing, especially in the morning. Allergies, sleep disruption and individual anatomy may contribute.

5. Mixed dark circles — Many patients have more than one cause, for example pigmentation plus a tear trough, or thin skin plus puffiness. These cases usually need a combination plan rather than one procedure.

Dark Circles: Cause and Treatment Approach

TypeWhat Usually Creates the DarknessTreatment Direction
PigmentaryBrown/grey-brown colour from melanin, rubbing, eczema or sun exposureSun protection, barrier repair, dermatologist-selected pigment-control skincare, selected superficial peels
Vascular / Thin SkinBlue-purple show-through from visible vessels and very thin skinSkin-quality treatments; selected vascular/energy options only after assessment
Structural / Tear TroughHollowing creates a shadow beneath the eyeSelected tear-trough filler, regenerative support or surgical assessment depending on anatomy
Puffiness / Eye BagsFluid retention or fat-pad prominence creates shadowingAddress contributing factors; selected non-surgical care or surgery if bags are structural
MixedTwo or more causes present togetherCombination treatment planned around the dominant causes

How Dark Circles Commonly Change With Age

Age influences skin thickness, collagen, facial volume and fat-pad position, but age alone should never determine treatment. A 22-year-old may have a prominent tear trough, while a 55-year-old may mainly have pigmentation. The table below describes common patterns, not fixed rules.

Age GroupWhat May Commonly ContributeTreatments That May Be Considered
Teens to Mid-20sGenetics, pigmentation, allergies/rubbing, thin skinSunscreen, barrier care, pigment-control skincare, selected gentle peels; treat allergy/eczema when relevant
Late 20s to Mid-30sPigmentation, thin skin, sleep disruption, eye strain/rubbing, early hollowingCause-specific skincare, skin-quality treatments; filler only if true structural hollowing is present
Mid-30s to Mid-40sIncreasing volume loss, fine lines, early laxity, mixed pigment and shadowSkin-quality or regenerative treatments, selected filler, and energy-based treatments depending on anatomy
50s and BeyondTear-trough hollowing, fat-pad changes, skin laxity, pigmentation and sun damageCombination treatment; selected filler/regenerative care or plastic-surgery assessment where appropriate

What Actually Works in Your 20s

For younger patients such as Aisha, pigmentation, genetics, allergies and repeated eye rubbing are common contributors. Treatment may include careful sun protection, barrier-supportive skincare and dermatologist-selected pigment-control actives. In suitable cases, gentle superficial peels or other skin-quality treatments may help.

Strong actives should not be used casually around the eyes because this skin is thin and easily irritated. Filler is not a treatment for pigmentation; it should only be considered when examination confirms a genuine structural hollow.

What Changes in Your 30s and 40s

In the 30s and 40s, several factors may begin to overlap. Sleep disruption, eye strain and repeated rubbing can make the area look more tired, while gradual collagen loss and early volume change can deepen the tear trough. At this stage, the question is no longer simply ‘how do we lighten the area?’ but ‘is the darkness coming from colour, skin quality, shadow, or a combination of all three?’

Depending on the diagnosis, treatment may include pigment control, skin-quality or regenerative treatments, carefully selected energy-based procedures, or conservative volume correction. The aim is to treat the cause rather than keep adding procedures that do not address it.

Why Sunita’s Case Needed a Different Approach

In Sunita’s case, age-related collagen loss and structural hollowing had become a major part of the problem. The resulting shadow made the under-eye area look darker even though pigment was not the only issue. This is the type of patient in whom a structural treatment may be considered after examining skin quality, tear-trough depth, cheek support and the presence or absence of under-eye bags.

Hormonal changes around and after menopause can contribute to changes in skin thickness and collagen, but they are only one part of the assessment. Treatment should still be based on the anatomy visible on examination.

Tear-Trough Fillers in Gurgaon: Benefits, Risks & Alternatives

Tear-trough filler can help selected patients whose main concern is true hollowing or volume loss. It does not treat pigmentation, allergy-related rubbing, prominent under-eye bags or significant skin laxity.

The under-eye is a technically demanding area. Inappropriate patient selection, product choice or placement can lead to persistent swelling, contour irregularity or a bluish Tyndall effect when filler is placed too superficially. Rare vascular complications are also possible. For this reason, under-eye filler should be treated as a medical procedure requiring careful anatomical assessment, not as a routine add-on.

Reassuringly, hyaluronic-acid filler placed under the eye can be dissolved with an enzyme (hyaluronidase) if the result is unsatisfactory — though avoiding the problem through correct assessment and placement is always preferable.

When Surgery May Be Better Than Filler

Some patients with significant lower-eyelid bags, excess skin, fat-pad prominence or advanced structural ageing are poor candidates for simply adding more filler. In these cases, a plastic-surgery assessment for options such as lower-eyelid blepharoplasty or fat repositioning may be more appropriate.

At Aesthetics Redefined by Cocoona, dermatology-led assessment can be coordinated directly with Dr Sanjay Parashar, Founder and Chairman of Cocoona Centre for Aesthetic Transformation, whose surgical experience includes lower-eyelid blepharoplasty and fat repositioning. When the problem is primarily structural, this coordinated approach avoids forcing every patient into an injectable solution when surgery may be the more logical, longer-lasting correction.

Where Regenerative Treatments Fit

PRP and other regenerative or skin-quality treatments may be considered in selected patients when thin skin, fine lines or overall tissue quality contribute to the tired appearance. They should not be presented as substitutes for volume correction when the main problem is a deep tear trough, or for surgery when significant bags or skin excess are present.

Newer options such as injectable skin boosters and polynucleotide treatments may also improve under-eye skin quality, hydration and fine lines in selected patients with thin or crepey under-eye skin. As with PRP, these support skin quality but do not replace volume correction for a deep tear trough or surgery for significant bags.

How Dr Kriti Lohia Assesses Dark Circles

At Aesthetics Redefined by Cocoona, Dr Kriti Lohia, Founder & Medical Director and an American Board Certified Regenerative Medicine Expert in Dermatology, evaluates dark circles by cause before recommending treatment. The assessment looks at the colour of the darkness, skin thickness, vascular visibility, tear-trough depth, under-eye bags or puffiness, pigmentation patterns, allergies or eczema, previous filler or procedures, and age-related facial volume changes.

This diagnostic approach is important because the same visible complaint can come from completely different structures. A pigment problem should not automatically be treated with filler, and a structural hollow should not be managed indefinitely with creams or home remedies. When several causes coexist, the plan may be staged so that each component is addressed safely.

Where the assessment points to a primarily structural or surgical cause — significant bags, excess skin or advanced hollowing — Dr Kriti Lohia coordinates directly with plastic surgeon Dr Sanjay Parashar, so you receive one integrated plan rather than a separate referral.

The under-eye area has thin skin, complex anatomy and little margin for error. A clinic should be able to explain what is causing the darkness, which treatment is expected to address that cause, what the limitations are, and when a procedure should not be done. The goal is not to choose the most popular treatment; it is to choose the least invasive treatment capable of addressing the actual problem.

If persistent dark circles are bothering you, book an under-eye assessment at Aesthetics Redefined by Cocoona, Golf Course Road, Gurgaon. Contact us to understand whether your concern is pigmentary, vascular, structural, puffiness-related or mixed — and which treatment, if any, is appropriate.

Dark circles are rarely just about sleep, and they are not one-size-fits-all. The most effective treatment begins by identifying what is actually creating the darkness before choosing a cream, peel, injectable, energy-based treatment or surgery.

People Also Ask

Why do I have dark circles even when I sleep well?

Sleep is only one factor. Genetics, pigmentation, thin skin, visible vessels, allergies, repeated rubbing, tear-trough hollowing and under-eye bags can all create a dark appearance even when sleep is adequate.

Are dark circles hereditary?

Yes, genetics can influence pigmentation, skin thickness, bone structure and the shape of the tear trough. A family tendency does not mean every family member needs the same treatment.

Can dark circles be permanently removed?

That depends on the cause. Some components can be improved substantially, while others may require ongoing maintenance as skin and facial anatomy continue to age. Complete permanent removal should not be promised without first identifying the cause.

Do home remedies like cucumber slices or cold spoons actually help?

Cold compresses may temporarily reduce puffiness by constricting superficial vessels, but they do not correct pigmentation, structural hollowing or significant skin laxity. Persistent dark circles need a cause-based assessment.

Is under-eye filler safe?

It can be appropriate in carefully selected patients with true tear-trough hollowing, but the area is technically demanding. Swelling, contour irregularity, the Tyndall effect and rare vascular complications are possible, so injector expertise and patient selection matter.

Can laser treatment remove dark circles?

Selected lasers or energy-based treatments may help certain pigmentary, vascular or skin-quality concerns, but they do not correct every type of dark circle and they cannot replace volume correction or surgery when the problem is structural.

Do dark circles get worse after menopause?

Age-related and hormonal changes can reduce collagen and alter skin thickness and facial volume, which may make under-eye hollowing or shadowing more visible. However, pigmentation, anatomy and other causes still need to be assessed separately.

What is the best treatment for hollow under-eyes?

The answer depends on tear-trough depth, cheek support, skin quality and whether under-eye bags are present. Selected patients may benefit from conservative filler or regenerative support, while others with significant structural change may be better assessed for surgery.

How much does dark-circle or under-eye treatment cost in Gurgaon?

It depends entirely on the cause and the treatment. Pigment-control skincare and peels sit at the lower end, tear-trough filler and regenerative treatments in the mid-range, and surgical correction such as blepharoplasty higher — with the exact figure confirmed only after a cause-based assessment. Be cautious of very low ‘package’ prices for under-eye filler, as this is a technically demanding area where expertise matters most.
